FMCSA Ends Paper DOT Medical Card Exception in 49 States on October 12

Starting October 12, 2026, CDL drivers in 49 states and Washington, D.C., will no longer be allowed to use a paper Medical Examiner's Certificate (MEC) as temporary proof of medical certification under a federal exemption.
FMCSA announced the change on October 8. Medical examination results are now transmitted electronically to state licensing agencies through the National Registry system. Drivers should make sure their medical certification is correctly recorded in their CDL records.
Alaska is the only exception. CDL and CLP holders licensed in Alaska may continue using a valid paper medical certificate for up to 60 days after issuance. FMCSA extended this exemption through October 12, 2027.
The change also affects trucking companies. Carriers outside Alaska can no longer rely on paper medical certificates under the expiring exemption and must verify drivers' medical qualification through the appropriate electronic records.
